Example List Request Form

Hi All,

My organization is looking to build a list/extraction request form that can be sent over to teams that have ad-hoc list requests. Would anyone out there have one built already?

Or are there any thoughts on what should be included? We would want it to include a few standard sections that ask what they need, the who, what, when, etc. I also want to build out a section that asks what types of outputs they need.

I'm thinking it would be best to build it in google docs or cognitio forms so that I can just send out a link for people to fill out.

Any suggestions or examples would be helpful. All of the form posts that would have helped ate 10+ years old and all the links are dead.

  • I don't have a specific example (am fortunate that my Dept power users are capable of pulling their own lists!), but if you have access, maybe if you look at how the bulk of lists your org has are constructed and kind of reverse-engineer a form?  As in, "these are our standard suppressions, this is how we define recent buyer", etc.  

  • I don't have a current one yet..new to Tessitura,  but if it helps, ones I have worked with in the past got a little to technical and lost adoption. 

    I would suggest a shared drive form with some generalized asks. One thing that I found helpful was asking them to define the use for the data and what they are trying to accomplish. 

    Sections on include, dates, exclude and displayed data are super useful.   Hope this helps. 

  • We are also in the middle of creating a list request form, so unfortunately I don't have any examples. One venue that seems valuable was making use of google forms and the ability to use conditional questions (someone requesting a list for solicitations for instance would have to answer different questions compared to someone requesting a single ticket buyers list). I would do as you mentioned and have a few standard sections, then begin to drill deeper in future sections. One thing to keep in mind is that with GF users can skips ahead to specific sections, but they can't skip ahead to specific questions, so I would just keep that in mind. If you or your team makes use of Trello, I would recommend using Jira forms, as ticket requests can automatically be linked as cards as new requests are made.
