My organization is looking to build a list/extraction request form that can be sent over to teams that have ad-hoc list requests. Would anyone out there have one built already?
Or are there any thoughts on what should be included? We would want it to include a few standard sections that ask what they need, the who, what, when, etc. I also want to build out a section that asks what types of outputs they need.
I'm thinking it would be best to build it in google docs or cognitio forms so that I can just send out a link for people to fill out.
Any suggestions or examples would be helpful. All of the form posts that would have helped ate 10+ years old and all the links are dead.

We have one built for development that we use Microsoft Forms for - Tessitura List Requests (office.com) (if you can't see that, please let me know and I can tell you the fields)
