My organization is looking to build a list/extraction request form that can be sent over to teams that have ad-hoc list requests. Would anyone out there have one built already?
Or are there any thoughts on what should be included? We would want it to include a few standard sections that ask what they need, the who, what, when, etc. I also want to build out a section that asks what types of outputs they need.
I'm thinking it would be best to build it in google docs or cognitio forms so that I can just send out a link for people to fill out.
Any suggestions or examples would be helpful. All of the form posts that would have helped ate 10+ years old and all the links are dead.

I don't have a specific example (am fortunate that my Dept power users are capable of pulling their own lists!), but if you have access, maybe if you look at how the bulk of lists your org has are constructed and kind of reverse-engineer a form? As in, "these are our standard suppressions, this is how we define recent buyer", etc.
