Online ADA seat purchase verbiage

We're getting ready to open our accessible seating for online purchases.  I think we need some language  that says something like "these seats should only be purchased by patrons with mobility issues".  Does anyone have such language they'd be willing to share?  I figure no sense in recreating the wheel...

    There's restrictions about what you can ask, but I'm not finding them in my notes right now.. I believe something along the lines of what is below is as specific as you can get.

    “Do you have a mobility disability or are you purchasing these tickets for an individual who has a mobility disability that requires the use of the accessible features that are provided in the accessible seating?”

    In other words you can't ask what specifically their disability is. We have our ADA compliant purchase path up and running for the SYOS part of our site right now. Best seat is coming when we get that 10.1 release.
