I can't fathom that this would be a known bug, it's so serious. I'm curious if anyone else can duplicate it or has seen it in their system.
It appears that when we extract constituents, and the output set (the default output set has these) contains phone number and email address, if the customer lacks a phone number, then the email address is also not output, even if it exists. We have no idea when this started happening or how long it has been going on, we just happened to have a couple of emergency communications to small audiences that were carefully scrutinized, which is the only reason we noticed the issue. I think there had been events in the past where people felt like the actual mailed numbers were too small, but it wasn't looked into thoroughly.

I’m about to go into testing of 15.1.4, we will try to add this to our testing cycle.

We have certainly placed a ticket, haven't checked the defect list.

Gawain Lavers and Karen SterkowiczGawain, Can you check something for me? On your Extraction, verify that Eaddress Marketing Indicator is unchecked (by default is is checked). If that is checked, then only Email Addresses with Allow Marketing checked will be included in the output. Since you are conducting an emergency communication you'd want Eaddress Marketing unchecked.

Interesting, for my first pass (a different extraction than the previous one we examined...) it does appear that there is a perfect match between customers who have phone numbers and customers who have "Allow Marketing" checked.
It does seem like it was a false correlation: there are simply very few customers with Market Ind checked and no phone number, and vanishingly few with a phone number and Market Ind not checked. And to be sure, unchecking the box causes all the email addresses to appear correctly. Thanks for figuring it out!
