Technology use survey?

Hello Tessi-friends:

As part of some strategic planning conversations we are interested in surveying our staff (either individually or departmentally) to find out:

• Their current use of and familiarity with our current technology resources
• Anticipated future technology needs (hardware, software, new systems, etc.)
• Any "what-if's" regarding their department's activities and how technology can play into that

Has anyone done anything similar within your organization? Did you find it to be effective? Does anyone have any survey questions they might be interested in sharing?
