When applying Hot Fixes, do organizations typically apply to test servers before applying to their production environment, or just go for it?

I always apply everything to TEST before applying to LIVE. Even if I'm not going to do a full suite of testing (based on the nature of the hotfix) it still makes sense to keep the instances in sync.
