How have other organizations used foursquare and other location-based social networking services? If so, how successful has it been? Any other ideas/opinions would be most appreciated.

At 92Y we give out eco-totes with our logo on it for anyone showing they have checked in to our box office staff. At our downtown NYC facility we give people a drink discount for checking in and showing the offer on their phones. We have had a few hundred check-ins at both locations. We find it a good way to build brand and not so much about driving direct sales.
