As you’ve not used this before it might be that some of the credit card types don’t have “Default for Billing” box checked in Payment Method setup. If that’s not it, for the problem ones have you tried re-associating a credit card to the Contribution?
If you still have no joy, you should submit a TASK ticket.

Hi, We are starting to explore using Pledge Billing. We've set this us in our test system, and have run the Pledge Billing Utility. However the Pledges were not paid, and are showing as 'Billed without Payment' in Controbutions. Running the Pledge Billing Summary, I can see that we have the following Billing Problem listed 'Could not find billing information for ref_no = xxxxxx' Has anyone come accross the issue and knows what we may have missed in our setup?
