How do you edit the Recorded field in plans to just say "Verbal".

I've successfully changed the field names in our plans, but when we pull an output set based on those fields, the field name reverts to the default. Is there a way to change the output field name?

I believe that what gets output on the Output Set is the Description field of TR_QUERY_ELEMENT. So, if you've made text changes to the user interface for your Plans, you'll probably also want to go into TR_QUERY_ELEMENT and update those Descriptions to match.
