Hi Megan, and Education community! I did my best to attend this meeting last week but was having WiFi issues, and was only able to pop in for 15-20 minutes or so. But I gained a lot of good info from the group about Jotform, and our academy is currently looking into using it for our Release and Authorization forms.
We currently collect some basic medical information from our dancers via paper forms - allergies, OTC medications they are allowed to take, and a section to write in any injuries or conditions that could be aggravated by the dance curriculum. Our manager is debating if HIPAA compliance is the best option or not. Would anyone out there who works with a school or academy be willing to share which plan they use, and why they chose it?
