I'm not sure if you have gotten feedback on this post. We do this at 92Y during the order process particularly for children's classes/events. I can put you in touch with our web manger if you have questions.
I'm a few years late to this conversation, but do you mind sharing your web contact at 92Y? We're dealing with a similar problem here at The Kentucky Center - we just launched a new website within the last year and the web is creating one-name households. :/
