I am in he process of giving our current renewal forms a make over for next season. If anyone feels comfortable sharing, I'd love to see examples you might be using!
The biggest thing (as I am sure a lot of us face) is it needing to collect a myriad of information from our subscribers, but needing it to look straight forward, simple and clear, while being informative.
I have a list of adjustments I am making moving forward, but always love to have input!

Are you looking for an order form that subscribers will fill out or that staff will fill out while talking to subscribers? How you lay it out and what details you ask for might be different in each case. I have an order form we use internally while taking orders as a backup to what goes into Tessitura and I'm happy to share if that's useful to you.
