Address Report with Ticket Information

I'm looking for a report that pulls:

Performance, Name, email address, full address, phone number, price type purchased, ticket quantity and total price paid.

Does anyone have a similar custom report? We do have a custom report that pulls all of this except for full addresses. Any suggestions for pulling this info would be helpful.

    Order Export Utility has all of that and a whole lot more.
