Do any of you have a standard imformation request template or form that people use when they need a list or a report done? How simple or detailed is the form? Would you be willing to share an example?

People here are meant to fill in a request via our request management system and what they are asked to supply there is:
Report Name
Purpose of Report
Required Search Parameters
Required Output (or attach an example)
However they very rarley do and I have to go and sit with them and work through it all with them.Caryl
I created a list request form that I make everyone fill out if they need to do a mailing and such. I'll post it on my profile page for you to take a look.

Oh. Watch out, the form I created is protected so people can only fill in the blank spaces, but you can remove the protection to update it as there is no password for the protection.
