I'm looking to add an icon the the area circled in red to alert an operators is there is an open CSI on account. I've seen some other organisations doing the same, but I cannot find anything that explains how.

Looks like you are utilising Flex Headers. Documentation for that is here https://www.tessituranetwork.com/Help_System/Content/Constituent%20Records%20Basic%20Features/Flex%20Headers.htm
You'd be looking at configuring TR_FLEX_HEADER_ELEMENT and TR_FLEX_HEADER in system tables.
