You might want to take a look at the Output Set Cookbook.
It doesn't have exactly what you are after, but I am sure there are examples that you can modify. If nothing else you will get an idea of what tables/views to use.
There is a link on this page to the cookbook for v12 - I am currently working on the v14 edition.

Hi everyone,
We are trying to pull membership info for members who have an 'inactive' membership as we want to do a big campaign contacting these people. Does anyone have any experience pulling this info (such as Most recent membership purchased/last active membership level; amount paid for that membership; affiliates in the household, etc...)? We have been told they need to be setup as custom elements...I am just not sure which table this info might reside in.
