I understand that the QR code format is hardcoded into the design for TRBO. Has anyone customized this design to print a Code 39 barcode with luck?

Not the exact solution you're looking for, probably, but if you know the value of the QR code as designed, you should be able to take that value and convert it into a Code 39 barcode. I've written about that process at some length here:

Our goal is to have our mobile sales team use TRBO for membership sales and print tickets that will serve as temporary member cards/passes for new members on that day.
Ideally, we would want TRBO to print a barcode that represents the new member's customer number (as is the case for our member cards), and is readable by both NSCAN (for member entry) and our ancillary systems so members can be validated and access their benefits/discounts while visiting.
Your document does give me some other ideas though as we think through the use of Activities in the future!
