We are attempting to convert our youth media releases to a digital form for greater ease of getting them to parents and back to us completed. I thought to query the community as to the best way to do this that is still legally binding. I’m wondering if we need to make sure I get a digital signature through use of an online site like docverify, or if simply using a Google or SurveyMonkey, or Wufoo form with a field to type their name and then a box to check would suffice. We’ve been Googling and haven’t been able to find a good answer for this. Especially as it relates to a general media release for minors. Any insights you have or directions you could point me would be greatly appreciated.

I may not be able to answer your question from a legal perspective but we've deal with the same issue and have started using smartwaiver.com as a solution. The entire process is digital which is really nice and patrons can download a PDF copy of their fully signed waiver for their records. It easily allows you to collect multiple signatures in one document (we have parents initial many points of policy) and allows you to safely collect minor signatures as well. They have several plan options and even a basic storage option for months when you're not collecting waivers regularly.
