Stock Gifts Custom Screen

We have a custom stock screen that we had gotten from another organization. The data entry is done after the contribution is entered and posted however, just as an FYI.

Does anyone have a custom screen for stock gifts they are willing to share a screen shot of? I've asked our IT department to create a custom screen which would include the following fields:

Date Received: date
Symbol: text
No. Shares: number
High: currency
Low: currency
Median : (calculated?)

I'm wondering whether  anyone has a working system.
