When you run this report, is it possible to have either the Affiliation Types or Association types set to "(none)"?
It would appear that if you run the report with the Affiliation Types selected as required but leave the Association types parameter box blank (because I don't want to include any), the blank is interpreted by the report as meaning "all" rather than "none". Attached is a screen grab of the report parameter screen and the report parameters at the head of the report.
I guess I COULD create a dummy Association and Affiliation that is never used so that I can at least limit the results but this doesn't seem like an ideal solution. Unless, of course someone can help by pointing out what I'm doing wrong!

I have noticed the same thing Martin. I think this might be one for a defect report ticket in TASK.
