Calling for MS Access help!

My predecessor created a front of house report that works by way of the front of house staff entering night of show observations into a MS Access form. That data gets saved in local tables in the impresario database, then they run an SSRS report that distributed the data from those tables (in a nice neat format) to internal stakeholders.

Our current Front of House Manager has asked me to add bunches of fields to this report, which first requires me to add them to the MS Access form. I am hitting speed bump after speed bump with this form. If anyone has Access form skills will you please reach out to me.
